脳出血の紹介

脳出血とは脳内の血管が何らかの原因で破れ、脳のなかに出血した状態をいいます。そのために意識障害、運動麻痺、感覚障害などの症状が現れます。MaxMan血腫が大きくなると脳浮腫によって頭蓋内圧が高くなって脳を起こし、重い場合は脳幹部が圧迫されて死に至ります。

近年、脳出血の死亡数は減ってきましたが、その最大の理由は広く行きわたりが十分に行われるようになったためと考えられます。また最近、脳出血は軽症化していますが、運動障害や痴呆などの後遺症で悩む患者さんが多いのも事実です。

脳出血が原因は多く、全体の７０％を占めます。血管の病変をみてみると、脳内の１００～３００μmの細い小動脈に血管壊死という動脈硬化を基盤とした病変ができ、これに伴ってできる小動脈瘤の破裂が脳出血の原因になります。RU486そのほか、脳動脈瘤、脳動静脈奇形の破綻、腫瘍内出血、脳の外傷、白血病などの血液疾患が原因になります。血管の壁に老人性変化のひとつであるアミロイドが沈着して脳出血の原因になることがあります。性脳出血を部門別にみてみると、最も頻度が高いのは被殻出血と視床出血で、この２つが約４分の３を占めます。次いで皮質下出血、橋出血、小脳出血、続きます。

脳出血の患者さんでは、意識障害とともに呼吸障害を伴う場合が多くみられます。倒れた直後に注意しなければならないのは、吐物によって窒息することと吐物を誤飲することです。吐いた場合は麻痺側を上に、顔と体を横にして誤飲を防ぎます。曲美救急車が来る前には、頭部を後屈させて下あごを持ち上げ、口を開けさせて気道を確保します。枕はあごが下がり、舌根が沈下しやすいので用いません。

　このような処置をして、患者さんはできるだけ早く運びを行うことが大切です。普段から高い患者さんに突然に起こる、上下肢における持続性で片側の脱力は、脳出血を含めた脳血管障害の可能性があるので、軽い場合でも神経内科、脳神経外科のある専門で精密検査することをすすめます。
